Obituary of Jeffrey R. Diaz De Leon
Jeffrey Diaz De Leon, age 45, passed away suddenly on Monday, April 11, 2022 at his home after a courageous battle with leukemia. Jeffrey was born on August 1, 1976 at the old Silver Cross Hospital, and had been a Joliet area resident his whole life. He was a huge Chicago Cubs fan. He loved to laugh, and enjoyed time with friends and family. Most importantly he was a dedicated husband and father, and will be greatly missed by all who knew him.
Jeffrey is survived by his loving wife of over 5 years, Raquel Diaz De Leon; his daughters, Alyssa Woodard and Julyssa Diaz De Leon; stepson Eugenio Salazar; his mother, Lola (Donald) Lewis; his father, Agustin (Laura) Hernandez; his brothers, Alfonzo, Carlos and Erik Hernandez; his sisters, Jeannie, Elvira, and Tara Marinez and Ashley Adams; and many nieces, nephews, aunts, uncles, cousins and good friends.
He is preceded in death by his grandparents, Angeline and Jose Reyes; and a brother in infancy, Jose Diaz De Leon.
All friends and relatives are invited to join in a celebration of Jeffrey’s life to take place on Monday, April 18, 2022 starting at 9:00 a.m. and concluding with services at 1:00 p.m. at the Fred C. Dames Funeral Home, 3200 Black Road, Joliet. Cremation rites will take place following services, and Jeffrey’s urn will be laid to rest near his family at Mt. Olivet Cemetery, Joliet at a later date.
